Please assist us by providing the following information prior to your consultation:
- Any recent X-rays or dental records from a previous dentist, if applicable
- A list of medications you are currently taking
- Updated dental insurance information to help us process your claim efficiently
- Signed release forms authorizing the transfer of records when needed
If you are transferring from another dental office, records and X-rays can typically be emailed directly to our office for your convenience.
Important: All patients under the age of 18 must be accompanied by a parent or guardian.
Please inform our office in advance if you have any medical conditions that may affect your treatment (e.g., diabetes, high blood pressure, artificial heart valves or joints, rheumatic fever, etc.) or if you are currently taking any medications (e.g., heart medications, aspirin, anticoagulant therapy, etc.).
After your appointment has been scheduled, electronic patient forms will be sent to you through Max Assist. Completing these forms in advance helps our team prepare for your visit and supports a more efficient check-in process upon arrival.
X-Rays
If your dentist has taken X-rays, you may request that they securely forward them to our office electronically. If additional imaging is required, it can be completed at our office during your visit.
